Memory Care Home Link
We understand that there is "no place like home" but with that comes unique challenges and needs. We help meet those needs by providing:
CASE MANAGEMENT - Family members work with a licensed professional who visits the client’s home to help problem solve ideas to help keep the client at home longer.
HOME ASSESSMENTS - Free in-home assessments completed by experienced professionals who assess the needs of both the individual and the needs of the caregivers.
COGNITIVE ASSESSMENTS - Assist with understanding the level of thinking, reasoning, and remembering of the person with memory loss. They also help the caregivers better understand abilities and limitations.
NETWORKING - Links caregivers with peers who are experiencing similar situations and are willing to share their stories and wisdom.
SUPPORT LINE - Talk in confidence with compassionate professionals who can offer caregiving advice, answer questions about dementia, and lend an understanding ear. Call 419-294-4973 and ask for Sandy Vent.
RESOURCE LISTING - An extensive list of area resources and contact information.
RESPITE CARE - Short-term stays in our Wetzel Memory Care program, based on assessment and availability. This service may or may not be covered by Medicare, Medicaid, or insurance and will incur a fee.
